How the exam comes together, probe to report
Start with a high-frequency linear head.Most interventional pain targets sit within 40 millimeters of the skin. A linear array at 10 MHz or higher separates the nerve from the tendon beside it, which a convex head blurs.

Survey the target, then set depth to it.Bring depth down until the target fills the upper half of the screen. The atlas frames on this page were captured at 20 mm and 40 mm, which is the working range for most blocks and joint injections.

Open the puncture guide and set the line.The guide draws the trajectory over the live image and prints its position in millimeters beside it. Out-of-plane mode gives a target circle with a radius instead of a line.

Advance in plane and watch the shaft.Keep the needle in the scan plane so the whole shaft stays bright, and confirm the tip before injecting. Color Doppler marks the vessel to avoid on the way in.

In 148 painful joints randomized to palpation or sonographic guidance, guidance produced 43.0% less procedural pain and 58.5% lower pain scores at two weeks.
Sibbitt et al., J Rheumatol 2009 - Median nerve area at the wrist has a published cutoff.
A meta-analysis of 28 trials and 3,995 wrists found 9 square millimeters the best single cutoff for median nerve area at the carpal tunnel inlet. Sensitivity was 87.3% and specificity 83.3%.
