How the exam comes together, probe to report
Start with the convex head in the flank.The kidneys sit deep and behind the ribs, so a convex array at 3.2 or 5.0 MHz is the head that reaches them. A linear head is too shallow for an adult kidney.

- Moderate hydronephrosis on point-of-care ultrasound is highly specific for a stone.
A meta-analysis of five high-quality studies covering 1,773 emergency patients found pooled sensitivity of 70.2% and specificity of 75.4% for nephrolithiasis. The finding of moderate or greater hydronephrosis reached 94.4% specificity.
