How the exam comes together, probe to report
Start with the convex head in the flank.The kidneys sit deep and behind the ribs, so a convex array at 3.2 or 5.0 MHz is the head that reaches them. A linear head is too shallow for an adult kidney.
Convex 3.2/5.0 MHz- Set depth at 160 mm, then open it if the kidney is deep.
Most adult kidneys sit inside a 160 mm field. The C5SH and D3Ultra open to 300 mm when body habitus or a high kidney needs it.
D: 160 mm on the header - Sweep the whole kidney before you call it.
A long-axis sweep from the upper pole to the lower pole is what finds a dilated collecting system. One frozen frame through the sinus can read as hydronephrosis in a kidney that is not obstructed.

The bladder answers outlet obstruction and post-void residual, and the vena cava reads volume status. The B4D reports bladder volume directly across a 50 to 2,000 ml range.

- Moderate hydronephrosis on point-of-care ultrasound is highly specific for a stone.
A meta-analysis of five high-quality studies covering 1,773 emergency patients found pooled sensitivity of 70.2% and specificity of 75.4% for nephrolithiasis. The finding of moderate or greater hydronephrosis reached 94.4% specificity.

A trial randomized 2,759 emergency patients with suspected nephrolithiasis to point-of-care ultrasonography, radiology ultrasonography or CT. Six-month cumulative radiation was significantly lower in both ultrasonography groups, and 30-day high-risk diagnoses with complications did not vary by imaging method.
Smith-Bindman et al., N Engl J Med 2014 - Lung congestion in hemodialysis patients is usually silent.
Among 392 hemodialysis patients scored by lung ultrasound, 45% had moderate-to-severe pulmonary congestion, and 71% of those were asymptomatic or had slight heart-failure symptoms. Very severe congestion carried a 4.2-fold adjusted risk of death.
