A neurology ultrasound machine images peripheral nerves and neck vessels with a high-frequency linear head, and reads flow through the temporal bone with a low-frequency sector head.

  • Nerve work runs on a linear head: the L14Pro images at 10/14 MHz with 256 elements and reaches 80 mm.
  • Carotid and vertebral studies use the same linear format at 7.5/10 MHz, where the D3Ultra reaches 100 mm.
  • The transtemporal window needs a low-frequency sector head, and the CD2 phased array runs 2.2/3.6 MHz to 190 mm.
  • Pulsed-wave Doppler, power Doppler, M-mode, calipers and DICOM export ship on every model on this page with no subscription.

Median nerve area at the wrist on the D3Ultra

Median nerve area at the wrist on the D3Ultra

The nerve is traced in short axis at the wrist and the app returns the area and circumference on screen. The frame reads 0.07 cm2, which sits under the 9.5 to 10.5 mm2 band pooled in the carpal tunnel meta-analysis below.

captured on a D3Ultra at H10.0 MHz, D 20 mm, GN 62 dB, DR 80, compound on. Area reads 0.07 cm2 and circumference 12.43 mm. Submitted by Enrique Botia Paniagua, 16 July 2026.
Interscalene brachial plexus on the D3Ultra

Interscalene brachial plexus on the D3Ultra

The nerve roots sit as dark rounded structures stacked between the anterior and middle scalene muscles. This is the window that follows an upper-limb nerve proximally when the lesion is not at the wrist or elbow.

Distal sciatic nerve in the posterior thigh

Distal sciatic nerve in the posterior thigh

The distal sciatic nerve is shown in short axis with the in-plane needle guide drawn across the frame. The guide reads its own position and angle, which is how an injection or a diagnostic block is planned.

Common carotid intima-media thickness on the D3Ultra

Common carotid intima-media thickness on the D3Ultra

The far wall of the distal common carotid is measured with a single caliper pair, reading 0.65 mm. Intima-media thickness is the first number a stroke clinic takes before it looks for plaque.

captured on a D3Ultra at 7.5 MHz, D 40 mm, GN 58 dB, DR 100, compound on; length 0.65 mm; submitted by Enrique Botia Paniagua, 16 July 2026
Right internal carotid stenosis from soft plaque

Right internal carotid stenosis from soft plaque

Power Doppler fills the residual lumen in red while the soft plaque stays gray beneath it. The caliper runs the length of the soft plaque and reads 20.41 mm, and the German on-screen note calls it a long soft plaque.

captured on a D3Ultra at H10.0 MHz, D 60 mm, PDI gain 100, steer 12.0, PRF 4.0 K, wall filter 15; length 20.41 mm; submitted by Dr. Walter Rebmann, 18 October 2025
Plaque measured inside the right internal carotid

Plaque measured inside the right internal carotid

The same artery in B-mode carries three caliper measurements across the stenosis region, at 4.58 mm, 8.97 mm and 8.44 mm. Measuring on a frozen frame is what turns a bedside look into a record entry.

captured on a D3Ultra at H10.0 MHz, D 40 mm, GN 70 dB, DR 70, compound on; on-screen note reads ACI re Stenosebereich; submitted by Dr. Walter Rebmann, 8 December 2025

Transtemporal midbrain with pulsed-wave Doppler of the right posterior cerebral artery

A 2.2 MHz sector head reaches the midbrain through the temporal bone at 130 mm depth, and the sample gate sits on the right posterior cerebral artery. The trace returns peak systolic 48.17 cm/s, end diastolic 22.32 cm/s, RI 0.54 and PI 0.83.

captured by a neurologist on an SD1 sector probe at 2.2 MHz, D 130 mm, GN 105 dB, DR 110. Gate 5 mm, angle -35 degrees, PRF 4.0 K. Submitted by Enrique Botia Paniagua, 16 July 2026. The SD1 is a TodoPocus-branded single sector probe and is not a Suresult catalog model.

Technique basics

How the exam comes together, probe to report

  1. Start on the linear head at 10 to 14 MHz.Start on the linear head at 10 to 14 MHz.

A peripheral nerve exam begins with the highest frequency that still reaches the nerve. The L14Pro runs 10/14 MHz over a 25 mm footprint, which suits the median nerve at the wrist and the ulnar nerve at the elbow.

    L14Pro: 10/14 MHz, depth 20/40/60/80 mm
  2. Trace the nerve in short axis and read the area.Trace the nerve in short axis and read the area.

Freeze the frame where the nerve is roundest, then trace inside the hyperechoic rim. The app returns cross-sectional area and circumference on screen, so the number goes into the note without a separate calculation.

    On screen: D 20 mm, F H10.0 MHz, area 0.07 cm2
  3. Follow the nerve proximally to the plexus.Follow the nerve proximally to the plexus.

Slide up the limb and keep the nerve in short axis until the roots appear between the scalene muscles. Comparing caliber along the course is what separates a focal entrapment from a diffuse neuropathy.

On screen: D 40 mm, GN 54 dB, DR 40
  4. Switch to Doppler for the neck vessels.Switch to Doppler for the neck vessels.

Power Doppler shows where flow stops at a plaque, and pulsed-wave Doppler puts a velocity on the narrowed segment. Both run on the same linear head that just imaged the nerve.

On screen: PDI gain 100, PRF 4.0 K, wall filter 15

What the research says

The evidence, four studies checked

  • Median nerve area near 10 mm2 pooled 84% sensitivity for carpal tunnel syndrome.

Across 11 of the 13 pooled studies, a median nerve cross-sectional area of 9.5 to 10.5 mm2 gave pooled sensitivity 0.84 and specificity 0.78. The authors placed sonography alongside electrodiagnostic testing rather than in place of it.

Descatha et al., Semin Arthritis Rheum 2012
  • A four-nerve ultrasound score separated CIDP from AIDP with 90% sensitivity.

The Bochum ultrasound score sums cross-sectional area at four sites, including the ulnar nerve in Guyon's canal and the sural nerve. Under blinded evaluation it distinguished CIDP from AIDP with 90% sensitivity and 90.4% specificity.

Kerasnoudis et al., Clin Neurophysiol 2014
  • A peak systolic velocity of 200 cm/s identified 70% carotid stenosis with 90% sensitivity.

A meta-analysis compared color duplex ultrasound with angiography in internal carotid stenosis. A peak systolic velocity of at least 200 cm/s detected 70% stenosis with 90% sensitivity and 94% specificity. At 130 cm/s for 50% stenosis, sensitivity was 98% and specificity 88%.

Jahromi et al., J Vasc Surg 2005
  • Optic nerve sheath ultrasound detected raised intracranial pressure with 90% pooled sensitivity.

A 2011 meta-analysis pooled six studies and 231 patients that compared optic nerve sheath ultrasound with invasive intracranial pressure monitoring. Pooled sensitivity was 0.90 and pooled specificity was 0.85.

What is the best handheld ultrasound for neurology?

For nerve work the Suresult L14Pro is the strongest fit, with 10/14 MHz on 256 elements and area calipers built into the app. For a clinic that also scans carotids and lungs, the D3Ultra carries linear, convex and phased formats in one probe.

Which probe format images peripheral nerves?

A linear array is the format for peripheral nerves, because nerves sit between 5 mm and 60 mm deep and need high frequency. Suresult linear models run from 7.5/10 MHz up to 18/24 MHz, and a nerve exam usually starts at 10 to 14 MHz.

Can a handheld ultrasound measure median nerve cross-sectional area?

Yes. A buyer capture on a Suresult D3Ultra shows a median nerve traced in short axis at the wrist. The app returned 0.07 cm2 of area and 12.43 mm of circumference on screen. The measurement is taken on a frozen frame and saved with the image.

Does nerve ultrasound replace nerve conduction studies?

No. A 2012 meta-analysis of 13 studies, 11 of them at the 9.5 to 10.5 mm2 threshold, examined median nerve cross-sectional area in carpal tunnel syndrome. Its authors concluded that sonography could not replace electrodiagnostic testing, but could give complementary results. Ultrasound adds structure and site of lesion where electrodiagnosis gives function.

How much does a neurology ultrasound machine cost?

A Suresult linear handheld for nerve work costs $2,139 to $3,720, with software, Doppler and DICOM export included. Clarius lists the L15 HD3 at $3,795 plus $695 per year, and Butterfly lists the iQ3 at $3,899 plus a plan from $299 per year.

Can a handheld ultrasound do a carotid duplex study?

Yes, within the limits of a focused study. The linear head carries B-mode, color, power and pulsed-wave Doppler. A 2005 meta-analysis found that a peak systolic velocity of at least 200 cm/s identified 70% stenosis with 90% sensitivity and 94% specificity.

Can a handheld ultrasound reach the transtemporal window?

Yes, with a sector head running near 2 MHz. The Suresult CD2 phased head runs 2.2/3.6 MHz to 190 mm, which is the format the transtemporal window asks for. A buyer capture on a 2.2 MHz sector probe shows the midbrain at 130 mm with a pulsed-wave trace of the right posterior cerebral artery.

Which Suresult model has the lowest sector frequency?

The CD2 has the lowest sector frequency published in the Suresult attribute tables. Its phased head runs 2.2/3.6 MHz to a depth of 190 mm, and it pairs with a 7.5/10 MHz linear head in the same 150 g body. The PD1 phased head runs 2.5/2.8/3.6 MHz, and the D3 family phased heads run 3.2/5.0 MHz.

Can a handheld measure optic nerve sheath diameter?

Optic nerve sheath diameter is measured on a high-frequency linear head about 3 mm behind the globe. Dubourg et al., Intensive Care Med 2011 pooled six studies and 231 patients, reporting 90% sensitivity and 85% specificity against invasive intracranial pressure monitoring. Orbital scanning should follow the ophthalmic acoustic output limits that apply where you practice.

Which frequency does small-nerve imaging need?

Digital nerves, the superficial radial nerve and small cutaneous branches sit within 10 mm to 20 mm of the skin. The L20Pro runs 16/20 MHz with 10 to 40 mm of depth, which is the band for those structures.

Does the scanner export a nerve study into the record?

Yes. Every Suresult model on this page saves stills and cine loops of 100 to 1000 frames, and exports JPEG, PNG, MP4 and DICOM. A nerve or carotid study reaches the record from the app without a cart system.

Is a subscription required to use the software?

No. Suresult software runs on iOS, Android and Windows with no annual fee. Color Doppler, pulsed-wave Doppler, measurements, needle guides and DICOM export are included on every model on this page.

Which model suits a neuromuscular clinic that also runs injections?

The L14Pro suits a neuromuscular clinic that also runs injections. It adds needle enhancement to the in-plane and out-of-plane guides, so a nerve is measured and then injected on the same probe. Buyer captures on a Suresult D3Ultra show the same guide overlay at the sciatic nerve and the adductor canal.

How long does the battery last on a clinic list?

The L14Pro holds a 4400 mAh battery rated for 4 hours of continuous scanning, and charges over USB or a wireless pad. The L10H holds 5600 mAh and is rated for 5 hours.
