Pleural effusion with atelectasis
An anechoic collection sits above the diaphragm with collapsed lung floating inside it. This is the finding that a supine chest film misses most often, and it decides whether a drain goes in.

A bright horizontal line with A-lines repeating below it is the normal aerated pattern. Gain and depth are set on this view before any anterior or posterior zone is scored.

Pericardial effusion on the D3Ultra
An anechoic rim surrounds the heart through the whole cardiac cycle. The buyer found it incidentally in a dialysis patient, and it is the ICU finding that most often changes management inside the hour.

The carotid artery and the internal jugular vein run across the field with parallel walls in long axis. This is the plane an in-plane needle follows, so the shaft and the tip stay in view during cannulation.

Technique basics
How the exam comes together, probe to report
Match the head to the ICU question.Cardiac windows sit between the ribs, so they need the phased sector. The convex head reaches the abdomen and the caval window, and the linear head images the pleural line and the neck vessels.

Set gain and depth on the pleural line first. Anterior zones separate A-lines from diffuse B-lines, and posterior zones find effusion and consolidation.

The phased head shows pump function and pericardial fluid at 160 mm to start. The convex head then measures caval diameter and collapse before a fluid bolus is committed.

In 32 patients with ARDS, lung ultrasonography was 93% accurate for pleural effusion and 97% for alveolar consolidation, against 47% and 75% for bedside chest radiography.
