Risposta rapida
A nerve block ultrasound machine is the imaging system that shows the target nerve and guides the needle to it in real time. It also confirms that local anesthetic has spread around the nerve. Dual-head wireless handhelds such as the Suresult D3Ultra now cover superficial plexus blocks and deep windows like the anterior sciatic, work that once needed a cart.
- Two heads: linear for superficial plexus, convex for deep sciatic windows
- On-screen needle guides: in-plane and out-of-plane, with position and angle readouts
- Image quality you can teach from: every block saved as cine loops and DICOM

The block atlas
Twelve nerve blocks, imaged on one handheld
“It is an excellent handheld ultrasound for performing regional anesthesia… I provide regional anesthesia for outpatient surgery, major surgery in hospital, in intensive care unit and in prehospital emergency medicine.”
— David Heimpel, who imaged all twelve blocks below on his D3UltraUnedited clinical captures · needle-guide readout on every frame · select any image to enlarge
Captured by a German anesthesiologist on his own device. The on-screen labels are German: Winkel = needle angle, Position = needle entry point relative to the probe, in mm.
Shoulder & arm1 block

Interscalene brachial plexus block
The roots between the scalenes: shoulder and upper-arm surgery. Superficial at 1–3 cm, a high-frequency linear view.
Chest & trunk4 blocks

PECS block
Pectoral nerves for breast and anterior chest wall surgery. Fascial planes across pec major, minor and serratus, on the linear head.

TAP block
Abdominal wall analgesia. The plane between internal oblique and transversus abdominis, on the linear head at the lateral abdomen.

Paravertebral block
Segmental thoracic analgesia beside the spine. The space lies deep to the transverse process and lateral to the vertebral body, within linear reach in most adults.

Erector spinae plane block
The ESP fascial plane: one level, wide thoracic spread. Deep to erector spinae at the transverse process. Linear or convex by habitus.
Hip & pelvis3 blocks

PENG block
Pericapsular nerve group for hip fracture and hip surgery. Between the AIIS and the iliopubic eminence, a deeper window that favors the convex head on larger patients.

Suprainguinal fascia iliaca block
The high fascia iliaca approach for hip fracture and hip surgery. Above the inguinal ligament, beneath the fascia iliaca in the iliac fossa; linear on most builds.

Proximal sciatic block (anterior)
The deep anterior window to the proximal sciatic nerve. The deepest window on this page at 6–10 cm, the convex head’s territory.
Knee & leg4 blocks

Femoral nerve block
Knee and thigh surgery; the nerve sits lateral to the femoral artery just below the inguinal ligament. Shallow, on the linear head.

Adductor canal / saphenous block
The saphenous nerve in the adductor canal: motor-sparing analgesia for knee surgery. Mid-thigh under sartorius, beside the femoral vessels, on the linear head.

Distal sciatic block
The sciatic nerve just before it splits into tibial and common peroneal, above the popliteal crease. Linear head at moderate depth.

IPACK block
Posterior knee analgesia after knee replacement. Local anesthetic is placed between the popliteal artery and the posterior capsule, at the level of the femoral condyles.

How ultrasound-guided nerve blocks come together on a handheld
Choose the head for the depth.Superficial plexus blocks live on the linear head; the deep sciatic windows belong to the convex head.
Linear and convex, one D3Ultra
Set depth and preset first.Start from the Nerve preset and set depth just past the target plane before the needle enters.

Keep the needle in plane.Advance along the probe’s long axis, keeping the tip in view; the on-screen guide and angle readout show the planned path.
Needle guide readout
Confirm the spread.Color Doppler before the pass shows any vessel in the needle path; local anesthetic should be seen spreading around the nerve.

What the research says
The evidence for ultrasound-guided nerve blocks
- Higher success, faster onset.
A 2009 British Journal of Anaesthesia meta-analysis found that ultrasound guidance cut block failure by more than half versus nerve stimulation (risk ratio 0.41). Onset was 29% faster and blocks lasted 25% longer.
Abrahams et al., Br J Anaesth 2009 - Fewer vascular punctures.
A 2015 Cochrane review of limb blocks found fewer accidental vascular punctures and less paresthesia with ultrasound guidance.
Lewis et al., Cochrane 2015 - Lower toxicity risk.
A 2013 registry study of 25,336 nerve blocks in Australia and New Zealand linked ultrasound guidance to a lower rate of local anesthetic systemic toxicity (LAST).

Before you buy
What to check before buying a nerve block ultrasound machine
What are the best ultrasound machines for anesthesiologists?
For daily regional anesthesia, the Suresult D3Ultra is the strongest fit. Its linear-plus-convex dual head imaged all twelve blocks in the Suresult atlas, from interscalene to deep sciatic. For superficial-only or pediatric blocks the 16/20 MHz L20Pro resolves finer detail, and the L10H covers standard plexus work at entry price.
Do I need a convex head for nerve blocks?
A convex head is needed for the deep windows: the anterior proximal sciatic sits beyond linear reach in most adults. That is why a dual-head machine matters for a complete block practice.
How much does a nerve block ultrasound machine cost?
A Suresult handheld nerve block ultrasound costs between $2,325 and $3,720, with software included and duties in the checkout price. Competing handhelds list at $3,595–$4,999 before subscription fees.
What is the real five-year cost?
Add the device price to five years of any membership fee, then add import duties if the list price is US-only. Clarius’s $595/year membership is $2,975 by year five; Butterfly’s required plan adds $1,495–$2,100.
Can I see the needle during the block?
Yes, with the needle kept in the probe’s plane. In-plane and out-of-plane guides draw the planned path over the live image, with entry-position and angle readouts. Every capture in the twelve-block atlas carries that overlay and its readout.
Is image quality good enough to teach with?
The twelve-block atlas is unedited. Fascial planes, plexus fascicles and the deep sciatic resolve clearly enough for trainee debriefs, and every image exports as DICOM or MP4 for the logbook.
Does it work for fascial plane blocks like ESP and TAP?
ESP and TAP blocks are both in the Suresult atlas. Fascial planes read best with steady 7.5/10 MHz imaging and a wide view of the muscle layers, which is the linear head’s territory. Larger patients may need the convex head for ESP.
What about blocks in the ICU or prehospital settings?
David Heimpel, who imaged this atlas, uses the same D3Ultra across outpatient surgery, major hospital surgery, intensive care and prehospital emergency medicine. The probe broadcasts its own Wi-Fi, so no clinic network is needed.
What frequency probe do nerve blocks need?
Most peripheral nerve blocks use a high-frequency linear probe: 7.5–10 MHz reaches plexus, fascial-plane and femoral targets at 2–4 cm. Blocks deeper than about 6 cm (the anterior proximal sciatic among them) need a 3.2–5 MHz convex head. That depth logic is why Suresult pairs both heads in the D3Ultra.
Is a handheld good enough for nerve blocks, or do I need a cart system?
For peripheral nerve and fascial plane blocks, a handheld images the same targets a cart does; the twelve-block Suresult atlas shows the D3Ultra doing it. A cart still earns its place for shared OR use, deep windows in large patients and advanced imaging modes. For anesthesiologists and CRNAs building a personal regional anesthesia setup, a dual-head handheld like the D3Ultra covers the daily case load.
